Note: During the night of 23rd October, 1941, the Cossack, while escorting a UK bound convoy from Gibraltar, was hit just forward of the bridge by a torpedo fired by U-563. Captain Berthon and 158 of his Officers and men were killed.

Note: H.M.S. Laforey was sunk north of Palermo, Italy, by a torpedo from U-223, commanded by Ober Lieutenant Peter Gerlach. Consequently, the u-boat was destroyed, according to German records, by depth charges from Destroyers H.M.S. Leander and H.M.S. Tumult and Destroyer Escorts H.M.S. Hambledon and Blencartha.

Ordinary Seaman P/JX 235487, H.M.S. Cossack, Royal Navy. Killed in action 23rd October, 1941. Age 19. Son of George and Mabel Alice Millard, of Brighton, Sussex. Commemorated on the Portsmouth Naval Memorial, Panel 51, Column 3.

Possibly Able Seaman, John Valentine, S.S. Fort Athabaska (London), Merchant Navy. Died 2nd December, 1943. Age 19. Commemorated on the Tower Hill Memorial, London, Panel 50.

Note: The merchant ship was lost in an air raid at Bari Docks, in Italy. In total, 17 ships were lost.  Several were carrying ammunition and blew up causing extensive damage to nearby ships.  Over 1000 men died as a result of the air attack. Fort Athabaska was a total loss, and 44 men out of her crew of 56 were killed.
